THIS IS AN IMPORTANT DOCUMENT - YOU SHOULD READ IT CAREFULLY BEFORE SIGNING IT. UPON SIGNATURE OF THIS FORM AND DECLARATION, YOU ARE ACCEPTING FULL RESPONSIBILITY FOR YOUR HEALTH AND MEDICAL CONDITIONS AND RELEASING 5TH POINT PTY LTD, THEIR PERSONNEL AND TRAINING INSTRUCTORS FROM ANY LIABILITY.

To perform the training requirements necessary for undertaking rope access manoeuvres, candidates should be physically fit and free from any medical conditions.

Declaration
I declare that I am in good health and physically fit to undertake the requirements of rope access manoeuvres. I do not have any medical conditions that may prevent me from completing the manoeuvres.

Risk and Disclaimer of Liability
I acknowledge and understand the potential risks of undertaking rope access manoeuvres and hereby release 5th Point Pty Ltd and the respective personnel from any and all liabilities (including financial loss), damage, personal injury (including physical and mental injury or incapacity), death or property damage, however caused, arising from my participation.

By signing this declaration, I acknowledge that I have read and understood its terms. I acknowledge that I am signing the agreement freely and voluntarily.

I consent to 5th Point taking photos and/or video recordings during my course. I understand that these photos may be used for promotional purposes, including being published on the website and/or social media.
